WHO ARE WE?

Dr. Rodziewicz specializes in surgery of the spine for adults age 18 and up. 

Dr. Rodziewicz is joined by Gregory Faltyn, PA.  Our PA acts as an extension of the doctor in practice philosophy as well as assisting in the operating room.

WHERE ARE WE LOCATED? 

CNY Spine Group is located at the Northeast Medical Center in Fayetteville, suite 217.  Enter the main entrance and take the elevators to the second floor.  Please see our Directions page for more information.

WHAT ARE THE OFFICE HOURS?

Monday through Friday, 8:00am - 5:00pm. 

HOW CAN I REACH THE OFFICE?

Our office phone number is (315) 637-7900 or 1-866-322-8877.  During regular office hours you may be asked to leave a message for the doctor.  Please remember that we are surgeons and may not be in the office every day.  After hours and on the weekends you will be directed to our answering service who will then contact the physician with any emergencies.

WHAT IF I CAN’T KEEP MY APPOINTMENT?

In order for us to best serve all of our patients, it is important that we stay on schedule.  If you are unable to keep a scheduled appointment, we request that you call and cancel at least 24 hours in advance.  This will allow us to offer an appointment to another patient.  If you arrive late for your appointment you may be asked to wait.  We also realize that your time is important.  Our practitioners will make every effort to keep on schedule, however please be aware that emergencies do occasionally arise which may result in your appointment being delayed or needing to be rescheduled. 

EFFECTIVE 6/1/12 NO SHOW OR SAME DAY CANCELLATION OF APPOINTMENTS WILL INCUR A $25 CHARGE NOT REIMBURSED THROUGH YOUR INSURANCE.



WHAT IF I NEED ADDITIONAL TESTING?

We will often schedule radiological testing for our patients.  We schedule MRI’s, CT scans and x-rays at the Northeast Medical Center, however if you prefer to have your testing done at another facility, we will make every attempt to accommodate your request. 

WHAT IF I NEED SURGERY?

Our practitioners operate at the Northeast Surgery Outpatient Center in Fayetteville, St. Joseph’s Hospital, Crouse Hospital in Syracuse and Upstate Orthopedics Ambulatory Surgery Center in East Syracuse.  Please let us know if you have a preference as to where you would like your surgery to be done.  Our staff will work with you to schedule your surgery for what is most convenient for you. 

WHAT IF I HAVE AN EMERGENCY?

Please contact the office as soon as possible so that your problem can be taken care of in a timely fashion.  Depending on the severity of your problem, you may be asked to come into the office to be seen or you may be directed to the Emergency Room. 

In addition, please understand that Dr. Rodziewicz is a solo medical practitioner, and therefore may not be personally available at all times. If you have a medical emergency arise when Dr. Rodziewicz is not available, you may be directed to a hospital emergency room or another neurosurgeon’s office.

INSURANCE/PAYMENT

Please be aware that because we are specialists, some insurance companies require us to have a referral to see you.  It is the responsibility of the patient to work with their primary care physician to make sure that this referral is in place before the scheduled appointment. 

While we participate with most major medical insurance plans, your entire visit may not be covered.  Co-payments are expected at the time of your visit.  Our billing department will work with your insurance company when possible, but ultimately the cost of services is the responsibility of the patient.  We accept Visa, MasterCard, Discover , cash and checks. 

PAIN MEDICATION POLICY

Pain medication is generally given for one month post-operatively only.  We ask that you work with your primary care physician for any other pain medication issues.  We may also refer you to a Pain Clinic for chronic or on-going pain issues.  

Pain medication renewals are done during normal business hours only.  There will be no pain medication renewals on the weekends, holidays, or evenings. 

DISABILITY DETERMINATION POLICY

                 •  Our expertise is evaluating and treating people for spine surgery. We will try to discharge other
                     obligations to the best of our ability.

                 •  We encourage you and your employer to discuss and agree to the kind of job you can perform
                     while under our care. We will approve any reasonable agreement you come to.

                 •  We have no expertise in occupational medicine. If we cannot answer questions related to
                     your job, we often suggest an Independent Medical Examination.


                •  Post-operatively, we typically give disability determinations for 90 days after surgery. Patients
                     needing further disability determinations are encouraged to obtain a Functional Capacity
                     Evaluation which will be signed by  one of  our practitioners.

                 •  Our involvement with your disability issues will end when you are discharged from our practice.
                     Please make sure that arrangements are made for continuing follow up if necessary.

                  

NO RECORDING POLICY

No recording of any nature, including but not limited to video, audio and still photos, shall be permitted on the premises of CNY Spine Group unless agreed to in writing by CNY Spine Group at least one day in advance.  All requests for an on-site recording should be submitted in writing to CNY Spine Group.  We will provide you a response to your request in a timely manner.  In addition, no recording of any CNY Spine Group personnel or other individuals on the premises, including other patients, shall be permitted without the consent of such person.

WHAT IF I HAVE FORMS TO BE FILLED OUT?

While we make every effort to fill out forms, we request that you give us 7 business days to complete them. As of 6/1/12 there will be a $10.00 charge for ALL disability forms. Payment will be expected at the time the forms are submitted to our office.
